29 CFR 1926.451(f)(3):  Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift,  and after any occurrence which could affect a scaffolds structural integrity:    a) The employer did not have a competent person inspect and ensure the 40 ft tall pump jack scaffold is safe for use; the scaffold was unstable due to insufficient bracing. Four employees on the scaffold at 25 ft elevation were exposed to scaffold collapse/ unstable scaffold hazard. 29 CFR 1926.452(j)(2):  Pump Jack scaffold poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled:     a) The 40 ft tall pump jack scaffold poles only were braced at the top of the 40 ft tall poles; scaffold was swaying excessively. Four employees on the scaffold at 25 ft elevation were exposed to scaffold collapse/ unstable scaffold hazard.
