29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about July 27 2019 at the LiquidPower Specialty Products Inc. (LSPI) facility, the employer did not develop, document, or use procedures to ensure the control of hazardous energy during servicing of a conveyor. Employees were exposed to caught-in hazards.

29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): On or about July 27 2019 at the LiquidPower Specialty Products Inc. (LSPI) facility, the employer did remove employees from the immediate area of a mill that was locked out, then re-energized for testing/debris removal. Employees were exposed to caught-in hazards.
