OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible dust deflagration, explosion, and other fire hazards from the deficient design and/or implementation of preventative and/or protective measures in the ACT Dust Collectors Model #ACT 3-18 Serial #15856: (a) Location - 300 Cedar Hill Street, Marlborough, MA: On or about 08/02/2019, air from the ACT Dust Collectors Model #ACT 3-18 Serial #15856 was exhausted to the production floor without the incorporation of provisions to prevent: 1. The recirculation of combustible paper dust from the dust collection system; 2. the propagation of flame and pressure effects should a deflagration occur in the air material separator; and 3. the transmission of smoke and flame should a fire occur in the dust collection system.
