29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about 2 August 2019, and at times prior thereto, electrical saws were not guarded at the point of operation, exposing the operators to an amputation hazard form the rotating blade. 1) Machine Shop: SOCO MC-275 Circular Cold Saw was missing the guard for the blade. 2) Machine Shop: Milwaukee Deep Cut Band Saw did not have guarding for the exposed portion of the blade. Additionally, the housing at each spool of the machine was open towards the operator.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  On or about 2 August 2019, and at times prior thereto, electrical extension cords and relocatable power taps were not being used in accordance with the manufacturer's instructions, exposing employees utilizing the equipment to an electrical hazard. 1) Office: Two relocatable power taps were connected in series. 2) Conference Room: An orange extension cord was plugged into the wall outlet, with a relocatable power tap connected to the female end of the extension cord in series. Connected to the relocatable power tap were two other relocatable power taps and a third connected in series. 3) Office - Warehouse Manager: An orange extension cord was plugged into the wall outlet, with a relocatable power tap connected to the female end of the extension cord in series. 4) Machine Shop: A relocatable power tap was zip-tied to a table leg, making it permanent wiring. 5) Logistics Department: Five relocatable power taps were zip-tied to either table legs, a U-channel, or a storage rack, making them permanent wiring. 6) Logistics Department: An orange extension cord was plugged into the wall outlet, with a relocatable power tap connected to the female end of the extension cord and a second relocatable power tap connected in series. 7) Logistics Department: An orange extension cord was plugged into the wall outlet, with a relocatable power tap connected to the female end of the extension cord in series. 8) Cleanroom: Two relocatable power taps were zip-tied to table legs, making them permanent wiring.
