29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:     a) On or about 7/26/19, at the site of Williamsville, NY. Employee, working from a scissor lift tightening metal clad wires to bar joists, did not put up mid chain of the basket platforms, and therefore did not install guardrails along all open sides and ends of platforms. Employee was exposed to fall of approximately 12 feet.     NO ABATEMENT CERTIFICATION REQUIRED
