Warehouse, Sort Desk: On or about August 6, 2019 and at times prior, the employer exposed employees to electrical hazards, in that an Underwriter's Laboratories listed, Staples relocatable power tap used to provide electricity to a Brother printer, a Motorola battery charger and a Symbol battery charger was not plugged directly into a branch circuit receptacle but was plugged into an extension cord which was plugged into a branch circuit receptacle.

Warehouse, JARS TD Inc work station: On or about August 6, 2019 and at times prior, the employer exposed employees of JARS TD Inc to electrical hazards, in that Amazon.com Services, Inc. provided JARS TD Inc with a work station having insufficient receptacles for needed equipment then permitted the ongoing use of electrical cords attached to surfaces with cable ties to provide electrical power for laptop computers, a printer and battery chargers. b. Warehouse, Express Parcel Service work station: On or about August 6, 2019 and at times prior, the employer exposed employees of EXPRESS PARCEL SERVICE LLC dba Express Parcel Service to electrical hazards, in that Amazon.com Services, Inc. provided Express Parcel Service with a work station having insufficient receptacles for needed equipment then permitted the ongoing use of electrical cords attached to surfaces with plastic cable ties to provide power to laptop computers, a printer and battery chargers.
