29 CFR 1910.23(c)(8) The employer did not ensure that the top cap and the top step of a step ladder were not used as steps: (a) This violation occurred on August 5, 2019 in the print and pack area where a Louisville, 8-foot, A-frame, self-supporting ladder was being used by employee(s) to clear jams in an overhead conveyor while standing on the top step of the ladder exposing employees to a fall hazard of approximately 8 feet to the concrete floor below.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) This violation occurred on July 29, 2019 when a transfer belt was being used on the conveyor feeding print and pack line number four with its belt un-guarded exposing employees un-jamming packages it to an ingoing nip point hazard.
