29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) DCM 3 - Employees working on DCM 3 were exposed to struck-by and caught-in hazards from performing maintenance on machinery without an energy control program, on or about August 6th, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: a) Panel HV-2 - The employees working near service panel HV-2 were exposed to electrical hazards from an open plug hole that was left open after a chiller removal, on or about August 6th, 2019. b) Panel LV-1 - The employees accessing service panel LV-1 to turn off tapper and lights were exposed to electrical hazards from open hole in top of service panel, on or about August 6th, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
