29 CFR 1910.268(g)(1): The employer did not ensure the proper use of a positioning system or a personal fall arrest system while work is performed at positions more than 4 feet (1.2 m) above the ground on poles and on towers: At a worksite located at 6409 Blueridge Cut Off, Raytown, MO: Employees were exposed to fall hazards. This was demonstrated on or about July 12, 2019 when an employee was observed climbing a utility pole (KCPL# RA254) with no means of positioning or fall protection system in place while accessing the pole.
