29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: a. Procedures did not accurately state what energy isolation device was used to de-energize machines, such as, but not limited to, the Accushear procedure, which stated that the disconnect on the machine and breakers were to be used for de-energization. The actual procedure was to de-energize at another disconnect next to the machine. b. Procedures did not accurately state how testing for de-energization was to be conducted, such as, when a voltmeter was not able to be used for testing, no alternative method was provided. Such as on saws, when a machine disconnect was used for lockout, a control button was not used for testing. c. There was no written procedure for the seamer table. (Note: actual shut down and lockout is to be done according to the steps stated on the written procedure.) Abatement documentation is required for this violation.

29 CFR 1910.140(c)(18): Personal fall protection system was not inspected before each work shift for mildew, wear, damage, and other deterioration, and defective components were not removed from service: An undersized pin was used on the vertical structural member on one side of the fall protection anchorage system with no slip pin (or cotter pin) to keep it in place. The correct size pin was used on the other side, but with no slip pin to keep it in place. Construction Trailer Specialists was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1333092, citation number 1, item number 1 and was affirmed as a final order on 9/10/2018, with respect to a workplace located at 2535 Rose Parkway, Sikeston, MO.
