29 CFR 1910.22(a)(2): (2) The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided. a) Main Storage Freezer, Access Door: On or about August 6, 2019, and at times prior, the employer exposed employees to slip, trip, and fall hazards, in that the main storage freezer lower door panel did not secure properly allowing air into the freezer resulting in a slippery ice buildup at the door threshold.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) Main Storage Freezer, Access Door: On or about August 6, 2019, and at times prior, the employer exposed employees to electrical shock and burn hazards, in that the main storage freezer door had exposed wires located at the bottom.
