29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation. On or about July 31, 2019, at the company establishment located at 15998 Walgreens Drive, Jupiter, FL 33478, the employer did not correctly record the following workplace injuries and illnesses on the OSHA 300 Log for calendar year 2019: a) The 7/24/2019 entry in Column F of the OSHA Form 300 for calendar year 2019 did not accurately describe the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill as required by the standard and listed in the instructions on the form. b) All entries in Column E did not identify where the event occurred or in the details as described in the instructions. c) The 7/24/2019 entry was incorrectly classified as an injury.
