29 CFR 1915.152(b)(4): The employer failed to document an occupational hazard assessment which should have included: Occupations involved in the survey, the date in which the survey took place and the name of the person who performed the hazard assessment: a) U.S. Coast Guard Response Boat 45621 (Shipyard/Wet Dock): On or about August 05, 2019, and at times prior; the employer failed to document that a workplace assessment to determine the hazards, and the necessary personal protective equipment required for the task had been performed.
