29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater: On or about August 6, 2019, the 480-volts disconnect panel for the conveyor belt from Cement Auger Bucket to the loading station was blocked with cartons of Fiberforce 300.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about August 6, 2019, an employee instructed to handle chemicals and clean the chemical storage area was not provided training on the hazards associated with the chemical products such as but not limited to Master Glenium 7920, Master Air AE90, and Fiberforce 300.
