29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)  846 E. Pikes Peak Ave, Colorado Springs CO 80903: On or about August 6, 2019, and at times prior, employees were exposed to struck by hazards while working below a scaffold without wearing hard hats.  Employees were installing framework for an elevator while another employee worked on a higher level of the scaffold.  None of the employees working on lower levels were wearing hard hats. 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:  a)  846 E. Pikes Peak Ave, Colorado Springs CO 80903:  On or about August 6, 2019, and at times prior, employees were not wearing eye protection while exposed to eye hazards.  Employees were using pneumatic nail guns to install the framework for an elevator shaft and were not wearing safety glasses. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:   a)  846 E. Pikes Peak Ave, Colorado Springs CO 80903:  On or about August 6, 2019, and at times prior, employees were exposed to fall hazards while accessing scaffold platforms more than 2 feet above a point of access.  Employees were using crossbraces as a means of access for scaffold platforms approximately 15 feet above the ground level. 29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph:  a)  846 E. Pikes Peak Ave, Colorado Springs CO 80903:  On or about August 6, 2019, and at times prior, employees were exposed to fall hazards due to guardrails not complying with the provisions in 29 CFR 1926.451(g)(i)-(xv).  The guardrails were incomplete or missing on the third level platform.  There were no midrails installed on the section of the platform parallel to the building and no guardrails on the left upper corner perpendicular to the building, exposing employees to fall hazards approximately 15 feet above lower levels. For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall:  a)  846 E. Pikes Peak Ave, Colorado Springs CO 80903:  On or about August 6, 2019, and at times prior, employees were exposed to struck by hazards due to lack of toeboards, screens, or other systems to catch or deflect falling objects.  Employees were working on a scaffold with three levels and none of the levels had protection from falling objects installed.
