29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about August 8, 2019, during the installation of framing and roofing material at the job site located at 6609 Birchbark Ave NE, Canton, Ohio 44721; the employer did not ensure employees working on the roof of a new construction single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material. The (Keim Builders) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(a)(1) Face and Eye Protection), which was contained in OSHA inspection number 1273772 citation number 01, item number 01 and was affirmed as a final order on (12/12/2017), with respect to a workplace located at 7203 Emerald Glen Ave NW, Canal Fulton, Ohio 44614.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 8, 2019, during the installation of framing and roofing material at the job site located at 6609 Birchbark Ave NE, Canton, Ohio 44721; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 14.3 feet by utilizing a fall protection system.
