29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a. On or about August 8, 2019, in the powder coat room, the employer did not ensure that a written respiratory protection program was established and implemented for those employees required to wear respiratory protection, including a 3M Versaflo TR-600 Powered Air Purifying Respirator, when performing powder coating tasks.  The respiratory protection program shall include, at a minimum, procedures for selecting respirators; medical evaluations; fit testing (for tight-fitting respirators); procedures for proper use in routine and reasonably foreseeable emergency situations; procedures for cleaning, storing, inspecting, repairing and discarding respirators; employee training regarding respiratory hazards they are exposed to, proper uses and limitations of respirators; and procedures for regularly evaluating the effectiveness of the respirator program.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:  Aspen Guard, LLC dba Folding Guard Company, Bedford Park, IL - On or about August 8, 2019, the employer failed to record the following workplace injuries in the detail required by the regulation on the OSHA 300 Log for calendar year 2019.  a. On or about August 2, 2019, a recordable injury occurred when an employee responded to a powder coat leak, resulting in a respiratory condition requiring hospitalization.  The information in Column F was not recorded in the detail required, as it did not describe the object that directly injured the employee.  b.  On or about July 11, 2019, a recordable injury occurred when an employee sprained the right wrist and sustained a lower back injury when tripping while climbing up stairs, resulting medical treatment beyond first aid.  The information in Column F was not recorded in the detail required, as it did not describe the object that directly injured the employee.  c.  On or about July 2, 2019, a recordable injury occurred when an employee sustained a lower back strain when moving a powder coat paint cartridge, resulting in three (3) days of lost time.  The information in Column F was not recorded in the detail required, as it did not describe the object that directly injured the employee.  d.  On or about May 5, 2019, a recordable injury occurred when an employee sprained the left index fining when it was caught in a side of a closing drawer on a tool caddy, resulting in 44 days of lost time.
