29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Lengacher's Framing & Finishing, LLC, on or about August 8th, 2019, at the location of 7034 Addison Place, Toledo, OH, 43617: The employer did not ensure that employees operating pneumatic nail guns to perform framing work were protected from potential flying particles striking the eyes through eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Lengacher's Framing & Finishing, LLC, on or about August 8th, 2019, at the location of 7034 Addison Place, Toledo, OH, 43617: Employees performing framing work on a new, residential structure were not protected from fall hazards of approximately 9 feet to the ground below.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  (a) Lengacher's Framing & Finishing, LLC, on or about August 8th, 2019, at the location of 7034 Addison Place, Toledo, OH, 43617: An employee performing residential sheeting work was using the sheeting material stacked on the forks of the JLG fork-lift as a temporary work platform without ensuring the platform was appropriately attached to the forks.  The employee was exposed to potential fall hazards of approximately 9 feet without fall protection.
