29 CFR 1926.502(f)(1)(ii): When mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8 m) from the roof edge which is parallel to the direction of mechanical equipment operation, and at least 10 feet (3.1 m) from the roof edge which is perpendicular to the direction of mechanical equipment operation. (a) Henry Gurtzweiler Inc., on or about August 8th, 2019, at the location of 8201 Central Avenue, Toledo, OH, 43617: The employer did not ensure that the warning line used on the flat roof was placed at the appropriate minimum distances from the edge of the roof while mechanical equipment was operated in a both a parallel and perpendicular direction. The employee using a walk behind cutting saw to remove the old membrane/insulation of the roof was exposed to fall hazards of 26 feet to the ground below without the benefit of fall protection.
