In accordance with 29 CFR 1903.19, abatement certification and documentation is required for this violation.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Detached Garage Roof, 233 Greenbush Street, Scranton, PA: Employees were engaged in roof replacement activities on a 4:12 pitched roof without any means of fall protection, exposing the employees to fall hazards of seven (7) foot, eight (8) inch from the roof eave to twelve (12) foot from the roof peak on the rake edge, as observed on or about August 8, 2019. In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a) Front Porch Roof, Residence, 233 Greenbush Street, Scranton, PA - Employees used the slide section of a 24-foot Lousiville Aluminum Extension Ladder for roof access/egress with side rails that extended six (6) inch above the top of the front porch roof eave exposing the employees to a ten (10) foot, eight (8) inch fall hazard, on or about August 8, 2019 b) Main Roof, Residence, 233 Greenbush Street, Scranton, PA - Employees used the fly section of a 28-foot Werner Aluminum Type III Extension Ladder - Model D1128-2 for roof access/egress with a right side rail that extended eighteen (18) inch and a left side rail that extended five (5) inch above the rake side main roof edge exposing the employees to a twenty (20) foot fall hazard, on or about August 8, 2019. In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): a) Detached Garage, 223 Greenbush Street, Scranton, PA - Employees on the detached garage roof used a closed 28-foot Werner Aluminum Extension Ladder for roof access/egress placed at a forty-six (46) degree angle such that the horizontal distance from the foot to the top support was seven (7) foot, two (2) inches which was roughly three-quarters of the ten (10) foot, six (6) inch working length of the ladder exposing employees to a fall hazard, as observed on or about August 8, 2019. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
