29 CFR 1910.268(i)(1): Head protection meeting the requirements of ANSI Z89.2-1971, "Safety Requirements for Industrial Protective Helmets for Electrical Workers, Class B" was not provided whenever there is exposure to possible high voltage electrical contact, and the employer did not ensure that the head protection was used by employees.   On or about August 1, 2019, at the worksite located across from 21209 Caris Rd. in Bowling Green, Ohio:  The employer did not ensure that aerial linemen utilized head protection to protect against contact with electrical conductors when working from an aerial lift to replace and repair utility poles and telecommunication strands located near a 7200 volt power source.

29 CFR 1910.268(j)(3): Vehicle-mounted elevating and rotating work platforms shall not be operated with any conductive part of the equipment closer to exposed energized power lines than the clearances set forth in Table R-2 of this section.   a. On or about August 1, 2019, at the worksite located across from 21209 Caris Rd. in Bowling Green, Ohio: A non-insulated vehicle-mounted (vehicle no. 219 1284) elevating and rotating work platform was operated closer than the minimum approach distance prescribed for electrical conductors with a 7200 phase to ground voltage.  Employees working to replace and repair equipment at the CenturyLink utility pole 606-8 were exposed to electrical hazards.     b. On or about August 1, 2019, at the worksite located across from 21209 Caris Rd. in Bowling Green, Ohio: A non-insulated vehicle-mounted (vehicle no. 219 1284) elevating and rotating work platform was operated closer than the minimum approach distance prescribed for electrical conductors with a 7200 phase to ground voltage.  Employees working to replace the CenturyLink utility pole 606-7 were exposed to electrical hazards.

An employee(s) was exposed to electrical hazards when guiding the pole to assist in placement next to power conductors.

On or about August 1, 2019, at the worksite located across from 21209 Caris Rd. in Bowling Green, Ohio: The employer did not ensure employees who are exposed to electrical hazards when replacing and repairing utility poles were trained in electrical safety-related work practices as required by 1910.331-1910.335.  Employees performing work near power conductors did not de-energize or guard the conductors, determine voltage, maintain the minimum approach distance, or utilize electrical personal protective equipment.
