On or about August 1, 2019, at the worksite located across from 21209 Caris Rd. in Bowling Green, Ohio: A non-insulated vehicle-mounted (vehicle no. 219 1284) elevating and rotating work platform was operated closer than the minimum approach distance prescribed for electrical conductors with a 7200 phase to ground voltage. Employees working to replace and repair equipment at the CenturyLink utility pole 606-8 were exposed to electrical hazards. b. On or about August 1, 2019, at the worksite located across from 21209 Caris Rd. in Bowling Green, Ohio: A non-insulated vehicle-mounted (vehicle no. 219 1284) elevating and rotating work platform was operated closer than the minimum approach distance prescribed for electrical conductors with a 7200 phase to ground voltage. Employees working to replace the CenturyLink utility pole 606-7 were exposed to electrical hazards.
