OSH ACT of 1970 Section 5(a)(l ): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following hazards: fall hazards while using a mobile ladder stand known as the "mortality cart." a. On or before August 8, 2019, the employer did not ensure employees who worked from the mortality cart were protected from falls while climbing rungs that were at least 16 inches wide and standing on a platform that was at least 18 inches wide. b. On or before August 8, 2019, the employer did not ensure that employees who climbed the mortality cart were protected from falls by providing climbing rungs with slip-resistant surfaces. c. On or before August 8, 2019, the employer did not ensure employees who worked from the platforms of the mortality cart were protected from falls by the use of a mid-rail.

OSH ACT of 1970 Section S(a)(l): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following hazards: electrical hazards while using a plastic welder and extension cord. a. On or before August 8, 2019, the employer did not ensure employees were protected from electrical contact hazards by removing an extension cord that was missing the ground prong and had damage to the outer sheathing. b. On or before August 8, 2019, the employer did not ensure employees were protected from electrical contact hazards by removing a cord and plug-connected plastic welder that had damage to the cord.

On or before August 8, 2019, where employees worked in house 7, the lower belt guard of row 6 was hanging free and missing screws, thereby exposing employees to caught-in hazards.
