29 CFR 1926.152(d)(2): A portable fire extinguisher having a rating of not less than 20-B units was not located between 25 feet and 75 feet from flammable liquid storage area(s) located outside: On or about 8/8/2019 at the Diesel tank all trades filling equipment including but not limited to powered industrial trucks were exposed to fire hazards where the employer failed to ensure there was a fire extinguisher present.

29 CFR 1926.152(e)(4): Units dispensing flammable or combustible liquids were not protected against collision damage: On or about 8/8/2019 at the Diesel tank all trades were exposed to fire hazards where the employer failed to ensure there collision protection in place. The tank was located in a heavy traffic area where vehicles including but not limited to powered industrial trucks, the water truck, crane, personal vehicles were driven.
