On or about August 08, 2019, and at times prior thereto:       a. Throughout the facility employees were operating a Nissan 50 forklift Model #: MP1F2A25LV that did not have restraint device.      b. Throughout the facility employees were operating a Nissan 60 forklift Serial #: E351259 that did not have restraint device.    c. Throughout the facility employees were operating a Hyster forklift Serial #: B0024D6857X that did not have restraint device.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed.    On or about August 08, 2019, and at times prior thereto, employees were exposed to caught-by hazards when working and walking near the chain and sprocket of the furnace D7 without a guard enclosing the chain and sprocket.
