29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about August 6, 2019, an employee was working without a hard hat on the ground level while an employee with a pneumatic nail gun and hand tools worked above him. This condition exposed the employee to struck by hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about August 6, 2019, the electrical extension cord that provided power to the circular saw did not provide for strain relief. This condition exposed employees to electrical hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 6, 2019, employees were performing residential framing construction on the top plate of a frame without fall protection. This condition exposed employees to the hazard of falling 12 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 6, 2019, employees used a six foot stepladder to access a ten foot frame. This condition exposed employees to the hazard of falling to the ground below.
