29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This was most recently documented on August 08, 2019 on the jobsite located at 3326 S 106th Street, Omaha, NE. Four employees were removing old shingles from of an existing residential house and were not utilizing any form of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a commercial roof.   This was most recently documented on August 08, 2019 on the jobsite located at 3326 S 106th Street, Omaha, NE. Four employees were using an aluminum extension ladder to enter and exit the roof of an existing residential house, where the side rails of the ladder were not extended to at least 3 feet above the landing surface which is about twenty-three feet above the ground.
