29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means    a. 288 Durant Ave., Staten Island, NY (Amboy Road Project)    The employer did not ensure employees determined the location of underground utilities in a safe and acceptable way.  Employees engaged in the installation of a storm drain chute, from the curb to the manhole in the middle of the road, struck a gas line with the excavator.     Violation occurred on or about August 9, 2019
