29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems, safety net systems, or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. 201 Howland Ave, Adams, MA: On August 7, 2019 and at times prior, an employee was exposed to fall hazards while working without fall protection from the roofs of tiny houses being manufactured. On August 7, 2019, that employee was injured after falling eleven feet from one of the roofs.

29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered, controls neutralized, and the brakes set to prevent movement. 201 Howland Ave, Adams, MA: On August 7, 2019 and at times prior, an employee dismounted a powered industrial truck with the forks raised and loaded with materials. On August 7, 2019, that dismounted operator employee was injured after falling eleven feet while unloading materials from the raised forks.
