29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:    (a) In the Cooking Connection area, mounted on the sole pillar near the entrance  (b) In the Business Center area, mounted on the exterior wall    On or about August 9th, 2019, and at times prior thereto, fire extinguishers were found throughout the facility that had not been inspected in over 1 year. This condition exposed employees to the hazard of fire.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:    On or about August 9th, 2019, and at times prior thereto, employees working in the Cooking Connection area had job duties that included cooking food items on a range and grill which generated carbon monoxide (CO). Employees were not trained to recognize that the vent hood positioned above the grill must be operating at all times when grilling food, exposing employees to the hazard of inhaling carbon monoxide.
