29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    Fire Riser Room, FACP, FDC; 10010 San Pedro office Building:  On or about August 08, 2019 and at times prior thereto, employees performing load bank testing on an energized alternating terminal switch were exposed to electrical hazards and were not provided training by the employer to ensure understanding of the type and magnitude of energy being worked with and the methods and means needed for energy isolation and control.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):      Fire Riser Room, FACP, FDC; 10010 San Pedro office building:  On or about August 08, 2019, and at times prior thereto, employees performing load bank testing on an alternating terminal switch were exposed to electrical hazards, and had not ensured the generator powering the alternating terminal switch had been effectively locked out and tagged prior to performing the load bank test.
