29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about August 09, 2019, at 194 SW Islewood Avenue, WESTON, FL, 33326, employees were exposed to a fall hazard of 14 feet to ground level while working on a roof with a 4:12 slope and not using any means of conventional fall protection systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 09, 2019, at 194 SW Islewood Avenue, WESTON, FL, 33326, employees were exposed to a fall hazard of 14 feet to ground level while gaining access to a roof of a residential home in construction with a ladder that did not have its side rails extended 3 feet above the upper landing surface of the roof and was not secured to prevent from displacement.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  On or about August 09, 2019, at 194 SW Islewood Avenue, Weston, FL, 33326, employees were exposed to a fall of approximately 14 feet to ground level when using a ladder set at an angle where the horizontal distance from the top support to the foot of the ladder was more than one-half of the working length of the ladder.
