29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: Location: 3514 Surf Avenue, Brooklyn, NY. First Floor Loading Dock. a) A wooden pallet was placed over a 3-foot by 3-foot floor hole that was not secured to the floor to prevent displacement. An employee fell through the hole approximately 12 feet to the cellar floor. On or about 08/10/2019.

29 CFR 1926.502(i)(4): All covers shall be color coded or they shall be marked with the word "HOLE" or "COVER" to provide warning of the hazard. Location: 3514 Surf Avenue, Brooklyn, NY. First Floor Loading Dock. a) Employees were working near floor holes that were not covered but were not marked to identify the hazard "Hole or Cover". An employee fell through the hole 12 feet to the cellar floor. On or about 08/10/2019.
