29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about 08/12/19, on jobsite at a house at 2167 Genesee St., Buffalo, NY:  Employees  used an extension ladder to access the "pik" of  a ladder jack scaffold erected on the east side of the house. The siderails of that extension ladder only extended approx.  20 inches above the edge of the pik.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a) On or about 08/12/19, on jobsite at a house at 2167 Genesee St., Buffalo, NY:  Two employees were exposed  to a potential fall of approx. 15 ft. from the pik on a Ladder Jack Scaffold erected on the east side of the house. The employees on the pik were removing the gutter. No Fall protection system was being utilized.   NO ABATEMENT DOCUMENTATION REQUIRED      Queen City Home Pros was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1248925, citation number 1, item number 2a, and was affirmed as a final order on 5/14/2018, with respect to a workplace located at 1529 South Park Avenue, Buffalo, NY 14220.
