On or about August 13, 2019 the electrical switch at the entry to the walk in cooler that illuminates the light inside the cooler had an incorrect switch plate cover that did not cover the entire switch exposing employees to electrical shock hazards.  b. On or about August 13, 2019 the electrical switch at the wall near the rear exit door, that illuminates the light at the rear foyer and the light at the exterior had a broken switch plate cover that did not cover the entire switches exposing employees to electrical shock hazards.
