29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person Location: Buildings G, I, and J Nancy Anne Lane, Nantucket, MA 02584 On or about July 11, 2019, frequent and regular inspections were not conducted to identify and correct struck-by, caught-between, and fall hazards in the following instances: 1) hard hats were not utilized while carpenters were working under overhead falling object hazards; 2) eye protection was not utilized while carpenters were operating pneumatic nail guns to install shingle siding; 3) carpenters were working under suspended modular home sections where rigging was not labeled to demonstrate knowledge of load capacity; 4) carpenters were working on a second floor residential deck where fall protection was not utilized; 5) a portable ladder was not extended at least 3 feet above the elevated working surface; 6) workers operating an all terrain forklift were not trained.
