29 CFR 1926.454(a)(2): Employees have not received training in the correct procedures for dealing with electrical hazards and for erecting, maintaining, disassembling the fall protection systems and falling object protection systems being used:  a) At the site: On or about July 29, 2019, employees installing conductive roof panels from scissor lifts were not provided training in procedures to prevent exposure to the electrical hazard posed by a 13,200 volt power line located less than 10 feet from the location they were to install steel conductive panels.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a) On or about July 29, 2019: At the site, an employee installing structural insulated panels at a roof peak with an unprotected edge approximately 25 feet above a concrete lower level was not protected from fall hazards by a fall protection system.
