The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:  a) Warehouse: On or about August 13, 2019, and at times prior, the employer exposed employees to struck-by and crushing hazards while moving burial vaults, in that an overhead crane (Konecranes Model CLX10C42200 Serial Number 10305145) was not being inspected frequently.

Complete inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated below. These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the following items. Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:  a) Warehouse: On or about August 13, 2019, and at times prior, the employer exposed employees to struck-by and crushing hazards while moving burial vaults, in that an overhead crane (Konecranes Model CLX10C42200 Serial Number 10305145) has never been completely inspected.

The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. Section 3.3.1 of the CGA Pamphlet P-1-1965 refers to storing cylinders in accordance with CGA and NFPA standards The CGA Pamphlet P-1-1991 Section 3.7.4.1 states that "All compressed gas cylinders in service or in storage shall be secured to prevent from falling."  a) Warehouse: On or about August 13, 2019, and at times prior, the employer exposed employees to struck-by hazards, in that an unsecured propane tank was stored in the warehouse.
