29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: (A) Guardrail systems; (B) Safety net systems; or (C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. On or about March 27, 2019, the employer required an employee to retrieve material from a storage rack measuring approximately 12 feet from the floor without wearing fall protection. The employee was exposed to a fall hazard. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about April 3, 2019, the employer did not train the employees on the safety and health hazards of chemicals they were required to work with such as, but not limited to, propane, flammable paint, and alcohol which can cause burns and skin irritation. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
