29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about 8/9/19, the employer failed to ensure that the lockout/tagout procedures were followed for the 7200 grinder, Model #CW1836, serial # RA-501.241593.10.1. A block was not utilized to prevent rotation of the grinder blades.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes. (a) On or about August 8, 2019 employees were exposed to a laceration hazard while cleaning out the 7200 Grinder without appropriate hand protection.
