29 CFR 1910.179(n)(1): Size of load. The crane shall not be loaded beyond its rated load except for test purposes as provided in paragraph (k) of this section. a)On 7/17/19 an employee was in the process of removing a support post assembly (1200 lbs.) from a welding work table utilizing a over head 3 ton gantry crane with a below the hook magnet attachment (660 lbs. max lift capacity.) During the lifting process from the table the support post detached and fell onto the employee's left foot. The support post exceeded the maximum weight capacity of the below the hook magnetic attachment by approximately 540 lbs.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On July 31, 2019, an employee sustained an amputation for a work related injury and the employer did not report a reportable injury to OSHA within the required twenty-four hours.
