29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:    a) On or about 08/14/2019 at 9776 Eagle Creek Center, Orlando, FL employees were working on the top and second level of the scaffold applying stucco on the building wall exposed to approximately a 23 and 14 foot fall hazard, in that, fall protection i.e. railings were not used.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:   a) On or about 08/14/2019 at 9776 Eagle Creek Center, Orlando FL, an employee was exposed to a 20 foot fall hazard, in that, no fall protection provided when working/standing on a unsecured platform attached to the forks of the United Rental JLG # 10136023 forklift operated by the team leader.
