The employer is failing to protect the employees from fall hazards due to the incorrect type of ladder being used to access the above break room area storage area.  A modified fiberglass ladder was found to be in use to access this storage area, and that the bracing side of the ladder had been removed, allowing ladder to be leaned and unsecured in nature.
