29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) On or about 08/14/2019, at the construction site, an employee working in an aerial lift at a height of 21 feet was utilizing a fall arrest system connected to a 23 foot rope with no deceleration device. The employee had not been trained on the correct way to setup a fall arrest system. b) On or about 08/14/2019, at the construction site, an employee working in an aerial lift at a height of 21 feet was wearing his harness' chest strap at his stomach. The employee had not been trained on how to wear the harness.

29 CFR 1926.502(d)(16)(ii): Personal fall arrest systems, when stopping a fall, did not limit maximum arresting force on an employee to 1800 pounds(8kn) when used with a body harness: (a) On or about 08/16/2019, at the construction site, employees performing scraping work in an aerial lift 21 feet above the ground were using full body harness which lacked deceleration devices to limit the arresting forces in the event of a fall.
