29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. On or about 14 August 2019, at a worksite located at 7721 N. Alpine Rd, Loves Park IL. 61111, employees were observed performing roofing operations on a commercial roof. The employees were exposed to live electrical power lines that were coming into the building from accidental contact when they were assigned the task of removing and replacing the roof. The employees were not protected from inadvertent contact with power circuits either by securing the power or from using insulation devices such as blankets or sheathing. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.502(f)(1): The warning line shall be erected around all sides of the roof work area. On or about 14 August 2019, at a worksite located at 7721 N. Alpine Rd, Loves Park IL. 61111, employees were observed performing roofing operations on a commercial roof, who were exposed to fall hazards from unprotected sides and/or edges which was in excess of 9 feet above the ground. The warning line system being utilized was not constructed around all edges of the building where employees were working and several of the employees were wearing fall protection harnesses but were not tied off. This exposed employees to a fall hazard from improper warning line setup. No other form of fall protection was utilized. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

On or about 14 August 2019, at a worksite located at 7721 N. Alpine Rd, Loves Park IL. 61111, employees were observed performing roofing operations on a commercial roof, who were exposed to fall hazards from unprotected sides and/or edges which was in excess of 9 feet above the ground. The warning lines that were being used were located less than the minimum 6 feet from the edge. This exposed the employees to a fall hazard in excess of 9 fee onto concrete. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

The materials that were stored on the roof were in close proximity to the edge to the edge of the building and within the 6 foot requirement. This exposed employees to a fall hazard in excess of 9 feet when retrieving material. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training. On or about 14 August 2019, at a worksite located at 7721 N. Alpine Rd, Loves Park IL. 61111, the employer failed to provide a written certification record of training that the employees had performed. Employees were performing roofing operations on a commercial roof, at elevated heights in excess of 9 feet, and were not adequately trained to recognize the hazards of falling and the steps to mitigate those hazards. This exposed employees to a fall hazard. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
