29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Worksite, 340 Blackheath Rd., Long Beach, NY- Employees were not wearing head protection while exposed to being struck by overhead materials such as, but not limited to hand tools and connecting hardware being used by workers overhead; on or about 8/14/20.   Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

The ladder siderails were not extended three feet above the upper landing; on or about 3/24/17. The foot of the ladder was approximately four feet from the supporting structure; on or about 8/14/19.
