C. Carlin Plumbing, Inc., 9744 Mark Road, Erie, PA  16509, on or about August 15, 2019 and times prior thereto:   Employees were working in a 63-foot long trench excavation that was at a depth of between 4.7-feet and 4.9-feet to perform work operations for the installation of a sewer line to a residence.  Stairways, ladders, ramps or other safe means of egress was not provided to the employees working in the trench excavation.

C. Carlin Plumbing, Inc., 9744 Mark Road, Erie, PA  16509, on or about August 15, 2019 and times prior thereto:    Employees were working in a trench excavation to perform work operations for the installation of a sewer line to a residence.  The spoil pile was within two feet and at edge of the trench excavation wall.

Carlin Plumbing, Inc., 9744 Mark Road, Erie, PA  16509, on or about August 15, 2019 and times prior thereto:  Employees were working in a trench excavation to perform work operations for the installation of a sewer line to a residence.  Employees did not have an understanding of the requirements set forth in 29 CFR 1926 Subpart P - "Excavations".       B.)  C. Carlin Plumbing, Inc., 9744 Mark Road, Erie, PA  16509, on or about August 15, 2019 and times prior thereto:  Employees were wearing personal protective equipment including, but not limited to, head protection, when working in a trench excavation to perform work operations for the installation of a sewer line to a residence.  Employees did not have personal protective equipment training.
