29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the construction site located at 1500 W Sunshine Street in Springfield, Missouri: Employee engaged in gutter installation activities was exposed to fall hazards of approximately 20 feet. Employees were observed accessing and egressing the roof by way of a portable ladder that did not extend 3 feet past landing.
