Mr. Bear Roofing was hired to perform roofing work at a multi-employer work site. Mr. Bear Roofing LLC had not developed and maintained a written hazard communication program. Hazardous chemicals present at the work site included, but were not limited to the following: 1. PVC and KEE HP Membrane Cleaner; and 2. Gasoline.

Employees installed roofing components, including the roof membrane, on the low slope roof. Employees worked in proximity to the roof edge. The employer utilized a warning line system that partially enclosed the work area. The partial warning line was erected less than six feet from the roof edge. This condition exposed employees to an approximate thirty-two foot fall hazard.

Employees installed roofing components, including the roof membrane, on the low slope roof. Work was taking place in the northeast corner of the roof. Employees accessed the roof via a fiberglass extension ladder on the south side of the building. The employer utilized a warning line system that partially enclosed the work area. Warning lines were not erected from the point of access several hundred feet to the area where employees were installing roofing. This condition exposed employees to an approximate thirty-two foot fall hazard.

Employees installed roofing components, including the roof membrane, on the low slope roof. The employer utilized a warning line system that partially enclosed the work area. The safety monitor participated in roofing installation work, in that he performed work outside the warning line and adjacent to the roof edge parapet. The employer did not ensure that the safety monitor did not have other responsibilities. This condition exposed employees to an approximate thirty-two foot fall hazard.
