On or about 08/14/2019, building F - an employee from BRR Construction Services, Inc climbed over the basket aerial lift to enter an open bottom catwalk like section to conduct framing work, exposing the employees to an approximate 18 foot fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a. On or about 08/14/2019, building F - personal fall protection was not attached to the basket anchor point and one employee was not using a harness while conducting framing work from an aerial lift, exposing the employees from BRR Construction Services, Inc to an ejection hazard. b. On or about 08/14/2019, building F - personal fall protection was not used by the employees from AIT Life Safety , while installing wet sprinkler systems from an aerial lift, exposing the employees to an ejection hazard. c. On or about 08/14/2019, building D - personal fall protection was not attached to the basket anchor point by the employees of Steel Worx LLC. In addition, an employee did not buckle the leg straps of the harness while conducting steel erection work from an aerial lift, exposing the employees to an ejection hazard.
