29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a. On or about 08/14/2019, Building E - Employees were exposed to an approximate 10 foot fall hazard while operating a JLG aerial lift while conducting steel erection work, in that, fall protection was not used.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of aerial lift being used and to understand the procedures to control or minimize those hazards:  a. On or about 08/14/2019, building E - an employee conducting steel erection work from an aerial lift was not trained in the recognition and avoidance of hazards associated with aerial lift usage. An employee was operating a JLG aerial lift  without buckling the leg straps of the harness while conducting steel erection work.

On or about 08/14/2019, at the job site - The OSHA 300 logs or copies of work-related injuries and illnesses were requested on 08/14/2019 and 01/08/2020. The employer failed to provide copies within 4 hours of request.
